TEACH Online Services Undergraduate and Graduate Waivers
Due to Family Education Rights and Privacy Act (FERPA), Fredonia needs authorization to release information in order to submit a teacher certification recommendation to New York State for all teacher candidates in Fredonia undergraduate and graduate teacher certification programs. If this waiver is not completed and submitted to Fredonia, a recommendation for certification will not be made. Below you will find the Google Form link for the Authorization to Release Information for Submission of Teacher Certification Recommendation (TEACH).
